Information about the above fields:

Last Name: Enter your Last Name (as registered with ASRM).

ASRM ID: Enter your nine-digit ASRM ID, as found on your Fertility & Sterility journal address label. If you ID number is not nine-digits, please add as many zeros as needed to the beginning of the number in order to make nine digits. Click here for a picture of the label containing the location of the ASRM ID#.

Remember Password: If you click on this field, the computer you are using will store your entered Last Name and ASRM ID in a "cookie" file. Storing this information on the computer, means you won't have to re-enter the information each time you wish to access the members-only area. Please note the following about using cookies:

The only information stored in the cookie file is your Last Name and ASRM ID. No other personal information is stored.
If you switch your computer and/or browser, you will have to re-enter your Last Name and ASRM ID again, because the new computer/browser won't have stored that information in its cookie file.
