• Registration & Housing Form (PDF): For the Scientific, Postgraduate, and Spouse/Guest Programs. You must register for the meeting in order to reserve housing.

• Team Member Registration Form (PDF): ASRM members attending the Annual Meeting may invite one member of their non-member medical team to attend the meeting for a reduced registration fee.

• Join ASRM - New Membership Form (PDF): If you are registering for the annual meeting and joining ASRM for the first time, please send registration and membership forms together to Experient if you are mailing or faxing them. This information is located on the registration forms. If you are not registering for the meeting, but would like to renew your ASRM membership, please fax or mail your dues directly to ASRM - Fax: 205-978-5005; Mail: ASRM, Attn: Membership Dept., 1209 Montgomery Highway, Birmingham, Alabama, 35216-2809.

8:00 am - 5:00 pm  Tuesday, October 16
8:00 am - 1:00 pm  Wednesday, October 17

Late Registration: Registration received after September 22 may not be processed in time for confirmation to be received by mail.  Online registrants will receive their confirmation by email or fax. 

Cancellation: Any cancellation/refund request will incur a $50 administrative fee.  After September 14 all fees are non-refundable.

Be Sure to Visit the Exhibit Hall

The Exhibit Hall will be open:
9:30 am - 5:00 pm  Monday, October 15
9:30 am - 5:00 pm  Tuesday, October 16
9:30 am - 2:00 pm  Wednesday, October 17

For the safety of your child and in order to maintain the scientific nature of the display, no children under the age of 16 (except infants under 6 months of age carried in arms at all times) will be allowed in the Exhibit Hall. Strollers and infants in backpacks are not permitted in the Exhibit Hall at anytime.
